Purpose
The purpose of this assessment task is to:
1.This assessment will provide an opportunity to research and consolidate the knowledge of selected nursing specialty topics gained in this unit and other units
2. This assessment will also build students’ literature review skills and reinforce the importance of evidence-based practice.
The Unit Learning Outcomes assessed are:
1. LO1: Articulate person-centred care to a range of common conditions across primary secondary and tertiary specialty areas including the pathophysiology assessment diagnosis and the management of these conditions across the lifespan;

3. LO3: Integrate evidence-based knowledge and give predict legal and ethical considerations when making clinical decision in relation to person-centred care scenarios across the lifespan with a range of related to selected specialty areas
Assessment Structure
Case Scenario :-
James Miller an 86 year old man with type 2 diabetes Mellitus osteoarthritis and hypertension has lived in an Aged Care Facility for three years. James was admitted to the local hospital one week ago for community acquired pneumonia and returned to the facility yesterday.
During his hospitalisation he experienced unstable blood glucose levels, so his oral hypoglycaemics were ceased and he was commenced on Lantus insulin 20 units nocte. James’s usual medications prior to admission to hospital included glipizide 10 mg and metformin 1,000 mg once a day. James was discharged to the aged care facility at 7.30 pm on a Saturday due to the need for his hospital bed. The medical officer completing the discharge medication form inadvertently included James’s usual oral hypoglycaemic medication in addition to the Lantus on the discharge medication chart.
On arrival at the Aged Care Facility, Anne Thomson the Registered Nurse looking after James read through his hospital discharge information and then filed it in his chart without ringing the General Practitioner and notifying him of James’s return to the facility and his current discharge medications. She did send the General Practitioner a fax informing him of James’s return but the GP would not receive this
until Monday. Anne had graduated one month ago and this was her first employment as a Registered Nurse.
She also did not identify that both James’s usual oral hypoglycaemics and Lantus were ordered and so she administered the oral hypoglycaemics as charted. Later that afternoon James was found to be cold clammy and unresponsive by nursing staff. James’s blood glucose level was found to be 2.1 mmol l and Anne organised for James to be urgently transported to hospital by ambulance.

1.As a student nurse you need to analyse the above scenario to identify the causes of medication errors and the best practice to prevent medication errors by referring to appropriate peer-reviewed evidence-
